Includes, but is not limited to: adjusted charges administrative and professional services for a simple cremation including 24 hour availability of transfer of the deceased (within 50 miles of Salt Lake City, Utah), basic staff services, refrigeration until permit is received, cremation, and preparation of death certificate. This charge does not include the use of facilities and staff for any visitations or ceremony, public or private, prior to the cremation.

- Utah Medical Examiner Permit - $150.00
- State Transit/Cremation Permit - $7.00
